He's saying they went with a simplified game plan that would allow them to think less and react more; i.e. "play fast," and it backfired because he overestimated their ability to win vanilla football. Overestimated their ability to win one-on-one matchups (raw talent).
“When you step back and you look at a number of performances that were very much subpar across the board, it’s not coincidence,” Campbell said. “And so that’s where, man, I did not do a good job of getting them ready. I did not, I did not. And it’d be one thing if there were just a couple of guys here or there, but ultimately, it was that I didn’t have them ready and I’ve got to do a much better job of getting them prepared ..."
"Our guys that get one-on-ones, our dudes, the guys that we really count on, when they get their one-on-ones, they’ve got to win them. But man, I am 100 percent responsible for that.”
“The plan, honestly, was simple. I thought it allowed them to play fast. We needed to challenge these receivers. Make him throw in tight windows and we called those, we just didn’t execute, we didn’t execute,” Campbell said.
“We need to get back to some of our fundamental work, our one-on-one work, our technique work,” Campbell said. “Good-on-good, just one-on-one pass rush. Good one-on-one man-to-man coverage, one-on-one getting open on routes, our releases, our handwork.

Sounds like coach-speak. Or at least MC/DC-speak, which is just coach-speak with a bunch of "man's" thrown in.
His coach-speak consists of blurting out exactly what he's thinking.
And then saying "and that's my fault."
Imagine if we could all just go around saying stuff like "Man, your kid is dumb. And your wife is ugly. But that's definitely my fault. I need to do better."

The game plan sucked on defense. And the offensive play calling was lethargic in the first few series.
So yes it is his fault but he put his guys in a bad position to begin with. Your asking LBs to cover WRs. The bad thing is they never adjusted on defense. Baltimore kept attacking the middle of the field and the soft zones on the outside. They were too concerned with keeping the QB in the pocket.
